Tax Forms
The Library has received form 1040 and 1040-SR booklets
We are awaiting delivery of IRS instruction booklets and do not have an estimated date of arrival for those. New York State tax forms and instructions should be shipped to us in mid February.
If there are forms that you need and are unable to locate online, please speak with a Reference Librarian. We would be happy to assist you!

With the ongoing COVID-19 pandemic and to stay in compliance with NY State guidelines, Bethpage has made the decision to go 100% virtual with Bethpage VITA in 2021. Unfortunately, this means that we will be unable to offer in-person VITA services at any of our previous VITA locations. However, we are hopeful that our new Bethpage Virtual VITA service will be able to assist many of the taxpayers in our communities.
Bethpage Virtual VITA services will be available to taxpayers by completing the “Bethpage Virtual VITA Application” that will help us collect the necessary taxpayer information and tax documents in order to complete tax returns through a virtual process.
Please note that the “Bethpage Virtual VITA Application” will be available to taxpayers as early as Friday, January 15th 2021.

HWCLI will begin contacting clients for an intake on February 1, but will not prepare or file returns until February 12.
